I'm coming off my 2nd ACL tear and am going to get fitted for a custom Ossur or Donjoy knee brace.  As for wearing it under the shin pad, do you just slap the shin pad over top?  Does anyone cut padding out of the shin pad so the shin pad can fit closer to the leg?  Curious if any of you have experience with this.  I wear 8k Pro shin pads so they are nice and wide, and don't have any straps so I'm used to fastening with just sock tape which I think will help if things fit a little more bulky.  Thanks in advance for any insight!

I wore a CTi brace for years (with a partially torn ACL) before eventually snapping the ACL completely and getting it completely replaced (wish I did that sooner).  When I had mine I was a lot more active and regularly mtn biked and snowboarded, I was able to add an optional knee pad that attached to my brace (had some padding and a plastic outer shell but wasn't much bigger than the brace so added minimal bulk).  I found that removing the knee liner/padding from my shin guards and using this knee pad on the brace worked perfectly for me since the pad part of the brace sat perfectly in the knee cup of the shin guards.  I do remember first trying without the knee pad attachment and having to trim some of the padding on the sides of the knee area on the shin guard to give me rom for the hinges (I've also seen a lot of pro stick shins that have been modified like that to fit a brace).  Honestly best bet is to just mess around with the brace on and the shin guard to see how you can make it work as best as possible.  Good luck!

Have you heard about Stoko? They are a tight that has wires running through them that lock your knee in place and are extremely low profile.

Evidently they are medical grade and can be covered by insurance, they show testing results at the same level (but not as high but close) to the full large custom braces.

I wore a Breg Axiom hinged knee brace under a Jofa shin pad with no liner.  It wasn't a perfect fit, but once I taped up it felt fine and didn't move around on me.  And it wasn't that visually noticeable, either. I think I skated a total of 6 times with it on before moving to a supportive knee sleeve. I used the sleeve for months before finally going naked about a month ago.
